Quarterly form filed by telecommunication companies to report their earnings.

A telecommunication corporation’s application for approval to offer, render, furnish, or supply telecommunication services to the public in Pennsylvania.

Telecommunication suppliers give contact information to the PUC on who to contact regarding such items as financial data, tariffs, customer service and annual reports. The form includes, name, title, address, phone numbers and email address for each contact.

Every jurisdictional telecommunications utility must complete and submit a Report of Outage Form to the Commission when they experience an unscheduled service interruption event that affects 2,500 or 5%, whichever is less, of their total customers for 6 or more consecutive hours. The Form is due within 10 working days after the last customer affected is restored. 52 Pa. Code Chapter 67.1.

All utilities under the reporting requirements of 52 Pa. Code §§ 57.47, 59.48, 61.28, 63.36, 65.19, 29.43, 31.1 or 33.103 shall annually file the form by February 28th. The PDF form may be filled out electronically, printed out (or printed out and filled in by hand), and must be mailed to Secretary of the Commission, Pennsylvania Public Utility Commission, Attention Angel Elwell, 400 North Street, Harrisburg, Pa. 17120. The forms may no longer be emailed to the Commission due to security concerns.

Utilities that have been directed to file an adoption supplement must file a separate form for each tariff adopted. The supplement may be filed on one days notice.

The PUC adopted a Public Utility Planning and Readiness Self Certification Rulemaking order at Docket L-00040166. The rulemaking resulted in the creation of a new regulation at 52 PA Code Chapter 101. The attached file contains answers to frequently asked questions concerning the form that is to be filed in compliance with 52 PA Code Chapter 101.4.

All local service providers intending to abandon local telephone service must fulfill the requirements at 52 Pa. Code, Chapter 63.306-308.

All telephone companies holding a Certificate of Public Convenience in the Commonwealth of Pennsylvania who wish to cancel their authority must file an application for approval to abandon or discontinue telecommunications services, in whole or inpart, in Pennsylvania with the Commission.

Under the Third Party Notification program, your utility company will notify you and another person you choose, to receive copies of shut off notices.

This form is to be used by telephone carriers that have been turned down for numbering resources by one of the numbering administrators. Please file this form and the appropriate attachments with the Secretary for docketing.

The descriptions of telecommunications carrier reports required by the PA PUC, and the associated PUC contact for that specific report. Each company is responsible to adhere to all of the established rules, regulations and directives of the Commission on reporting requirements of utilities.

Fixed Utilities Statement of Operating Revenues for General Assessment for Calendar Year 20XX (Jan – Dec 20XX).  Reports are due by March 31st of each year to report revenue for the prior Calendar Year.

That for the period of July 1, 2023, through June 30, 2024, the TRS surcharge rate for residential and business access lines shall be set at $0.00 per month, unless the Commission takes further action to revise the TRS surcharge rate prior to June 30, 2024.  (see Docket No. M-2023-3037441 Order entered, May 18, 2023.)  For this period the telecommunications companies are not required to submit a $0.00 remittance form.

Interconnected VoIP providers must utilize this form to notify the PUC of its intent to request numbering resources from the Numbering Administrators.

An annual report by all ILECs and CLECs that contains residential account information for the previous year as listed in 52 Pa. Code § 64.201. The report is due within 90 days after the end of each calendar year.

Due by March 31, 2024. The Annual Tracking Report should be mailed to or eFiled with the Secretary of the Pennsylvania Public Utility Commission, at 400 North Street, Harrisburg, PA 17120.
