Instructions for Tyler Insurance Filings - Instructions for submitting Insurance Forms E, H and K. 

Filing ​of Forms and Bonds

Certificates of insurance are not binding and therefore are not acceptable. Motor Carriers are not able to provide binding proof of insurance to the Commission. ONLY insurance companies can provide the required forms to the Commission.

Form E

Binding evidence of motor carrier bodily injury and property damage liability certificate of insurance. Here is an example of Form E.

Form H

Binding uniform motor carrier cargo certificate of insurance.

Your insurance company should subscribe to Tyler Insurance Filings. (www.tylerinsurancefilings.com). You will request the insurance company to file the required insurance forms electronically through Tyler Insurance Filings. A time period of 60 days is given to have binding insurance proof filed with the Commission. Applications will be dismissed when insurance agents do not arrange for this evidence to be filed in the allotted time period. A Certificate of Public Convenience (CPC) will not be issued, and a new application will have to be submitted with another application fee. The electronically filed insurance forms will reach the Commission more quickly than mailed forms.

Surety Bond

Evidence of surety is needed for brokerage authority only. Choose between a continuous bond and a fixed-term bond. An original bond must be mailed to the Commission.

Cargo Waiver

Should you be eligible for an exemption from cargo insurance, you will need to submit an Exemption from PUC Cargo Insurance Regulations cargo waiver form to the Commission instead of a Form H being submitted by your insurance company.

Financial Responsibility of Carriers (Minimum Limits)

Passenger Carriers

15 Passengers or less

  • $35,000 minimum split coverage - split in the amounts of $15,000 bodily injury per person, $30,000 bodily injury per accident and $5,000 property damage per accident (52 Pa. Code §32.11(b)).
  • First party coverage of the driver of certificated vehicles shall meet the requirements of 75 Pa. C.S. 1711(relating to required benefits).
16 to 28 Passengers
29 Passengers or more

Property Carriers

Bodily Injury

  • Not less than $300,000 per accident for a vehicle with a manufacturer's gross vehicle weight rating of 10,000 pounds or less, in the case of a single vehicle, or a manufacturer's gross combination weight rating of 10,000 pounds or less, in the case of an articulated vehicle. The liability of the insurance company on each motor vehicle operated in common or contract carrier service shall be in amounts not less than $750,000 per accident for a vehicle with a manufacturer's gross vehicle weight rating over 10,000 pounds, in the case of a single vehicle, or a manufacturer's gross combination weight rating over 10,000 pounds, in the case of an articulated vehicle. 52 Pa. Code 32.11(d) and shall meet the requirements of 75 Pa. C.S. 1701-1799.7.
