SHE TRANSFERRED THE SERVICE FROM HER ADDRESS TO ANOTHER ON THE DAY SHE MOVED. NEVERTHELESS, PGW BY MISTAKE KEPT ON BILLING HER FOR THE OLD ADDRESS. IN THE MEANTIME SHE DID NOT RECEIVE A BILL FOR THE NEW ADDRESS. SHE WOULD LIKE TO DISCUSS THE MONTHLY PAYMENTS AND MAKE SURE THAT THEY EXIST AND STAY THE SAME AFTER SHE SPEAK WITH PGW REPS. SHE WOULD ALSO LIKE THE LATE CHARGES TO BE WAIVED IN THE AMOUNT OF $20 FOR EACH MONTH.

HE WANTS THE PUC TO ORDER THE PGW TO (1) PRODUCE THE MONTH AND AMOUNT FOR EACH PAYMENT NOT RECEIVED, (2) CREDIT THE REFERENCED ACCOUNT FOR ALL PAYMENTS IT DID RECEIVE, (3) STOP ALL COLLECTION ACTIVITIES AGAINST THIS ACCOUNT, AND (4) TAKE ALL OF THE NECESSARY STEPS TO CLEAR HIS NAME AND ACCOUNT OF ALL NEGATIVE REPORTS TO THE CREDIT BUREAU(S).
