New Application Forms - Please discard any old application forms and instructions and replace with the new forms and instructions that have a revised date of October 5, 2021 or newer. Beginning January 1, 2022, older versions of applications will no longer be accepted.

Interstate Motor Carriers - The PUC does not have jurisdiction over interstate motor carriers. If your service territory involves crossing state lines, you are not required to file an application for authority with the Commission. Please see our exemption list for other possible exemptions from PA PUC Authority.

Motor Carrier - Ongoing Applications Downloadable Forms

All PUC motor carriers must submit a change of address to the Public Utility Commission whenever their contact information changes. Keeping the PUC informed will ensure documents sent by the Commission will be received. Government documents will not be forwarded by the USPS, they will ONLY be returned.

Overview of the safety regulations for carriers of people, property and household goods between points in Pennsylvania.

Allows carriers of property, when transporting certain bulk commodities, when providing service exclusively in dump trucks, or when transporting shipments that do not exceed $500 in value, to waive the Commission's requirement to carry cargo liability insurance.

All utilities under the reporting requirements of 52 Pa. Code §§ 57.47, 59.48, 61.28, 63.36, 65.19, 29.43, 31.1 or 33.103 shall annually file the form by February 28th. The PDF form may be filled out electronically, printed out (or printed out and filled in by hand), and must be mailed to Secretary of the Commission, Pennsylvania Public Utility Commission, Attention Angie Elwell, 400 North Street, Harrisburg, Pa. 17120. The forms may no longer be emailed to the Commission due to security concerns.

The PUC adopted a Public Utility Planning and Readiness Self Certification Rulemaking order at Docket L-00040166. The rulemaking resulted in the creation of a new regulation at 52 PA Code Chapter 101. The attached file contains answers to frequently asked questions concerning the form that is to be filed in compliance with 52 PA Code Chapter 101.4.

Form that gives a list of items that should be done prior to signing any agreements with a moving company.

An application must be filed for any public utility or an affiliated interest of a public utility prior to its selling or transferring capital stock to any person, partnership or corporation. Also, an application must be filed for any public utility to acquire 5% or more of the voting capital stock of any corporation.

This form is used to provide the Bureau of Investigation and Enforcement with a list of the vehicles being used in taxi and limo transportation. It should be completed and submitted (between December 1st and December 31st of each year) in fulfillment of the compliance requirements and in compliance with 52 Pa. Code §29.314(c) and §29.333(d).

Statement of Operating Revenues for General Assessment of Common Carriers of Property and/or Persons by Motor Vehicle for Calendar Year 20XX (Jan – Dec 20XX).  Reports are due by March 31st of each year to report revenue for the prior Calendar Year.

The regulations regarding the change in a carrier’s name, entity, and/or fictitious name can be found at 52 Pa. Code 3.381.  These instructions have been prepared in order to summarize the requirements.

Cancellation and Discontinuance Applications

Public Utility Commission motor carrier authorities must notify the Commission before closing down operations. Failure to notify the Commission can result in fines or administrative holds on further registration or transfer of vehicles in some cases.

Instructions and applications in this section are used to cancel and discontinue an existing PUC authority. Abandonment and discontinuance of service, which is published in the PA Bulletin, is required of airport transfer, brokers of household goods, brokers of passengers, call or demand, group and party carriers of 11-15, household goods in use, limousine, paratransit and scheduled route carriers. These abandonment applications are published in the PA Bulletin.

Cancellations are used for property carriers, and group and party carriers of 16+.

Motor Carrier - Cancellation/Discontinuance Applications Downloadable Forms

Instructions and application to abandon or discontinue service, in whole or in part, between points in the Commonwealth of Pennsylvania.

Carriers must use this form to cancel their property carrier operating authority with the PUC.

Group and Party (Passenger 16 +) Carriers must use this form to cancel their passenger carrier operating authority with the PUC. This form is ONLY for vehicles that have a seating capacity of 16 or more, including the driver.
